Hair transplantation : Microsurgery: redistribution of one's own hair

Usually hereditary baldness occurs at the frontal sides and on top of the head.

So the hair at the back of the head can serve as donor hair. During surgery a flap of skin, maximum 16 cm by 1,5 cm is removed. A local anaesthetic is applied to the scalp. The procedure lasts between 3 and 6 hours.     

Hair transplantation: Microsurgery: the best technique

• 1. Under visual magnification, the hair follicles are individually harvested from the donor area. This is performed by our team of well trained doctors and nurses.
• 2. Micro-surgical dissection of the donor strip must be perfomed in a professional manner: this is essential for successful transplantation.
• 3. The harvested donor strip is cut into small, single units made up of one or more follicles. Single follicle units are  transplanted at the frontal hairline. Multiple follicle units (2 to 4 follicles) are used on the top of the scalp to provide a denser effect.
• 4. The hair surgeon implants each graft individually into the balding scalp. The procedure is extremely labour intensive. It requires 3 to 5 medical and paramedical specialists throughout the 2 to 6 hour surgical procedure.

Wat is Follicular Unit Extraction (FUE)?

FUE hair transplantation is a technique whereby follicular units are removed from the donor area using a small punch instrument. A surgical punch is a hollow micro-needle with a diameter of 0,8 to 1 mm.

The FUE method: first the hair surgeon selects the follicular unit. Then he makes a punch hole and pulls the entire follicle away from the hairy scalp. The harvested follicular units are transplanted one by one into small holes in the bald area using a small tweezers.

The only advantage of this technique is that the donor material can be inserted without the need for stitches. This is easy for the surgeon, and the technique can therefore be performed by doctors with a minimum of experience in cosmetic suturing techniques.

One disadvantage is that a limited number of follicular units can be transplanted per session. FUE hair transplantation is only suitable for treating small areas. The treatment is very time consuming and therefore less comfortable for the patient.

The main disadvantage is that the donor side - the back of the patient's head - is full of needle holes, which later form visible scars.

Laser Micrografting

The hole in which the follicle is transplanted can be made in one of two ways:

• by making a small incision with a surgical scalpel (= PRICK method),
• or by burning a hole with a laser

The laser burns result in tissue loss around the transplantation holes. This makes the technique unsuitable for closing the existing head of hair. This is why at the Wellness Kliniek we prefer the better, more labour intensive pin-prick method, for a fuller head of hair.

Why is the microsurgical technique better?

Microsurgery requires a more specialised approach. More hairs (up to 3000 !) can be transplanted during the same procedure.

Any scarring is barely visible. Use of the 'Trichophytic Closure technique' means that the incision is stitched back together so closely that later it is extremely difficult to detect any evidence of hair transplantation surgery.

Scar revision following FUT hair transplantation surgery

Scar revision following FUT hair transplantation surgery, not performed in our clinic, is a very common procedure. Many patients who have had this treatment elsewhere come to us with this problem.

By using the special Trichophytic Closure method the hair surgeon can remove the scars and then perform the suture in an extremely fine line, which is practically invisible in time. The operation is performed under a local anaesthetic. The patient can leave the clinic again shortly after the procedure.

Clones - Stem cells: Hair transplantation for the future

Cloning is hot! Hair cloning means multiplication of the transplants. Stem cells take on the properties of hair follicles. It could be the solution for baldness in the future.

Cloning: a great deal of scientific research is being carried out on cloning tissue wth stem cells, also for follicle cells. Cloning follicle tissue is not going to be possible tomorrow. Only the future will reveal if this technique is reliable and whether the results are permanent.

In hair transplantation surgery there is a certain type of cloning: when you divide a hair in two the you can make more hairs grow out of the same follicle that there were originally. There are 'more' hairs but they are finer in structure than the original hair.

 They don't provide enough volume! This is why the technique is not recommended for head hair transplantation.

Micro-grafts are meticulously inserted

Depending on the number of hairs per unit, this can be the equivalent of 3000 to 4500 single hairs!

The scar at the donor site - a straight line - is hidden among the remaining hairs. Thanks to new surgical closure techniques - trichophytic closure-, the fine line scar will be practically invisible after a few months.

After surgery, the small wounds will develop a little scab. After about ten days the scabs will disappear. You will notice hair growth straight away, however this is only temporary.
